Release of Medical Records

To access a copy of your medical records, please print and complete the . You must mail, fax, or email the authorization form to any of the following addresses. Be sure to keep a copy for your records:

ImmuNet 

ImmuNet is Maryland鈥檚 Immunization Information System, a secure web-based registry operated by the Center for Immunization at the Maryland Department of Health (MDH). It stores an individual鈥檚 vaccination records and is a web-based tool for healthcare providers and schools to keep their patients/students vaccinated on time and avoid being under or over-vaccinated.  ImmuNet information is confidential, HIPAA and FERPA compliant, and available only to authorized users and will not be released to third parties without written consent. The Health Center receives vaccination records from ImmuNet electronically and the Health Center sends information on administered vaccines to ImmuNet electronically.

If you do not want to disclose your immunization information to authorized users of ImmuNet, you may opt out at any time by completing an Opt-Out Form. Should you decide later to rescind this opt-out and have your information made available to your health care provider(s) in ImmuNet, you must complete a Rescind Opt-Out Form.
